Rehabilitation Services
Eastwood proudly collaborates with U.P. Rehab Services that have several services to accommodate almost any physical need.
Physical Therapy offered up to five days a week focuses primarily on transfers, ambulation, endurance, and gait training.
Occupational Therapy offered up to five days a week focuses on dressing, fine motor skills, upper body strengthening, and cooking skills.
Speech Therapy focuses on cognitive skills training, speech and swallowing difficulties.
Orthopedic doctor visits three times a week to ensure your well managed to achieve your goals!
Skilled Nursing
We provide 24 hour nursing care for all our residents. Throughout days and nights, our staff is composed of LPN and RN nurses for medical management, and Certified Nurses assistants who assist for activities of daily living.
Dietary Management
We provide a spectrum of dietary services that individual to each resident including weight management, diet management and education. Our dietary staff strives to provide you with diverse, home-cooked and heart-warming dishes that foster memories of the good ol’ days.
Skilled Wound Care & Rehab
Dedicated for residents who are in need of skin related issues, our wound care department are highly recognized for their treatment and healing for even severely complex wounds! Also provided are exercise and walking programs for residents with skilled staff to ensure residents maintain physical functions, Available seven days a week.
Activities Department
We provide an eclectic, well-rounded, person-centered activities program that is individualized for each resident. At the heart of Eastwood, we strive to provide social and emotional health. Many exciting activities that are offered every day; we strongly encourage our residents and families to join!
Social Services
Available for residents and families for assisting in social, psychological, and emotional needs, social services are essential in your transition into Eastwood and your journey back into the community. Offering many resources, social services are the point of contact for you and your family to address and questions you may have. This department is responsible for any services that you may need during your stay.
